Office visit and surgery charges are determined by the time spent and by the severity and complexity of the problem. Fees are slightly higher for your first visit as it requires more time to evaluate a new patient, make out a chart, and obtain your old records.
We charge a minimum $25 for any letter or form filled out that is not directly related to insurance billing or referrals, such as letters to the Jury Commissioner, DMV or Disability forms. A stamped self addressed envelope is requested with all such forms. |
