Appointments may be made by calling 541-1177 during regular office hours.  The best time to call is in the afternoon from 2:00 to 4:30 P.M.  Please inform the receptionist of the reason for the visit to ensure that enough time can be scheduled for your needs.  A limited number of appointments are available each day for same day callers with urgent problems.  If you find that you will be late or unable to keep your appointment, please notify our office in advance to allow us to ensure prompt appointments and accomodate emergencies. If you are a new patient, you may print out and fill out a patient information form and history form. Please print out these pages (acrobat pdf, 167 kb), fill them out, and bring them with you to your appointment.
